Checklist and Enrollment process

Here is the checklist of things you will need to register your student:

Birth Certificate

Immunizations

Proof of Residency

Voluntary $5.00 Snack Fee

Please bring these items to school!

• Eligibility: Your child must turn five before September 1st of the enrolling year.
• Required Documents: Parents/Guardians must provide the following during enrollment:
  • An official birth certificate
  • An immunization record or Utah exemption form
  • Proof of residency (must include address, parent name, and current date)
• Enrollment Application: Enrollment is done online at https://www.canyonsdistrict.org/depts/admissions/new-student/
  • If you already have a student attending in the Canyons School District, click on “New Student Enrollment” to link your new student with your existing Skyward account.
  • If you are new to our district, click on “Temporary Account for New Student Enrollment” to begin the process.
